Social scholarships - documents

The student and each adult member of the student's family applying for financial aid shall submit:

  1. A certificate of income subject to personal income tax under the rules set forth in Articles 27, 30b, 30c, 30e and 30f of the Personal Income Tax Act of July 26, 1991 for the student and each of the adult members of the student's family, earned in the calendar year that precedes the year of application, issued by the head of the competent tax office, which includes information on the amount of:
    a) income,
    b) social security contributions deducted from income,
    c) tax due.

The student attaches appropriate certificates from the relevant tax office on income or lack of income separately for each adult member of the student's family;

  1. certificates from the Social Insurance Institution regarding the student and members of the student's family, including information on the amount of health insurance premiums paid for all adult members of the student's family who earned income last year;
  2. statements of the student and each adult member of the student's family regarding income not subject to income tax for the previous calendar year (appendix No. 1 to the application for a social grant).
  1. IMPORTANT!!! If the monthly per capita income in the student's family has not exceeded the amount specified in Article 8, paragraph 1, point 2 of the Act of March 12, 2004 on Social Welfare ( currently 600.00 ) the student is required to attach to the application for a social grant a certificate from the Social Welfare Center or the Center for Social Services on the income and asset situation of his/her and the family.
    This certificate must be current and should cover the period from the year preceding the academic year in which the student applies for the benefit to the present.
    In the case of failure to attach to the application a certificate from OPS / CUS or attaching a certificate from which the actual income and property situation of the student's family does not appear, and its content is limited only to stating the fact of NO BENEFIT from social assistance UKS is obliged to refuse to grant a social grant to such a student, unless the student documents the sources of family support. In such a case, the student attaches a Statement on the financial situation of the student and the student's family (Appendix No. 2 to the application for a social grant).

Additionally:

If the student or members of the student's family are engaged in business under the provisions of the flat-rate income tax on certain income earned by individuals or in the form of a tax card, it is necessary to provide:

a certificate from the competent tax office regarding family members who settle their accounts on the basis of the provisions on flat-rate income tax on certain income earned by natural persons, containing information on:

a) form of tax paid,
b) amount of income,
c) tax rate,
d) amount of tax paid;

If the student or members of the student's family own a farm, you must provide:

  1. A certificate from the municipality on the number of converted hectares or a payment order,
  2. a lease agreement, concluded in accordance with the provisions on social insurance for farmers, in the case of leasing part or all of an agricultural farm owned by the student's or student's family,
  3. a lease agreement concluded in the form of a notarial deed in the case of contribution of a farm for use by an agricultural production cooperative,
  4. a lease agreement in the case of the lease of an agricultural holding in connection with the collection of an annuity specified in the regulations on support for rural development from funds originating from the Guarantee Section of the European Agricultural Guidance and Guarantee Fund and in the regulations on support for rural development with the participation of funds from the European Agricultural Fund for Rural Development

In determining the family's income derived from the farm, the area of the farm used as the basis for agricultural taxation shall include the agricultural areas leased with the exception of the agricultural areas specified above in items 2, 3, 4.

If the student or members of the student's family receive income and benefits abroad, you must provide:

relevant documents issued by foreign equivalents of Polish offices and institutions, containing data analogous to those required for income earned in the country. The above-mentioned documents should be translated into Polish;

If the student or members of the student's family pay or receive alimony, you must provide:

  1. a copy of a copy of a court judgment awarding alimony to persons within or outside the family, or a copy of a copy of the minutes of a meeting containing the contents of a court settlement, or a copy of a copy of a court-approved settlement reached before a mediator,
  2. money orders or money transfers documenting the actual amount of alimony received, as well as a bailiff's certificate on the total or partial ineffectiveness of alimony enforcement, as well as on the amount of enforced alimony in the case of obtaining alimony lower than that awarded in a judgment or court settlement or settlement before a mediator,
  3. the decision of the competent authority granting the alimony advance with a determination of its amount,
  4. decision of the Alimony Fund on the allocation of Fund benefits and their monthly amount,
  5. A copy of the judgment dismissing the alimony claim,
  6. a copy of the final court decision obliging one of the parents to bear the entire cost of supporting the child

If the student or members of the student's family declare a loss or gain of income, it is necessary to provide:

  1. request for recalculation of income,
  2. documents confirming the loss of income specifying the amount of income lost,
  3. Employment contract/contract of employment and a document stating the amount of net income earned from the first full month worked by the student or a member of his/her family (a contract of employment is not earned or lost income)

If there are unemployed persons in the student's family, it is necessary to provide:

certificate from the Labor Office from unemployed persons confirming the status of unemployed and the amount of unemployment benefit received (net) or a statement from persons not registered with the above-mentioned office.

If the student has siblings or children of their own, it is necessary to provide:

  1. An abbreviated copy of the child's birth certificate or a complete birth certificate for siblings or children under the age of 7,
  2. certificate of attendance at school or college of the applicant's siblings or children up to the age of 26,
  3. information of the court about pending proceedings for adoption or adoption of a child, in the case of a person actually caring for a child who has applied for adoption of that child,
  4. a copy of the final judgment of the family court stating the adoption of the child,
  5. A certificate of disability in case the family is raising a disabled child regardless of age.

Other documents:

  1. death certificate of a deceased family member,
  2. decision(s) on the granting of a survivor pension,
  3. a copy of a final court judgment declaring a divorce or separation

A student applying for material aid of a social nature shall submit the necessary original documents or their certified copies. A copy of a document may be certified by an employee of the Dean's Office, a notary or the institution that issued the document.
